Services We Offer

Additional Information:

"When you or your loved ones are counting the moments, count on us."

Our professional staff is available 24 hours a day, 7 days a week in your home, in nursing homes, in assisted living residences and in our in-patient care facility, offering our patients "seamless care." Essentially, we provide care in nearly any environment the patient calls home.

The Hospice of Saint John tailors its care to the individualized needs of our patients and their families.

Our admissions team will guide patients through the admissions process and provide assistance by answering all of your questions. We will help you make decisions regarding your hospice care. You are not alone in this process.

Our professional comfort care teams coordinate care with patients, families, loved ones and primary care physicians.

Each Comfort Care Team consists of the following:

Attending Physician:

You can choose to have your primary care physician follow your care or the Hospice of Saint John can provide our own physicians. Our medical directors will review your plan-of-care regularly with you, your family and your interdisciplinary team.

Expert Nurses:

Our nurses are skilled experts in assessing the needs of hospice patients to help develop individualized care plans.

The nurse's primary goal is to provide pain, symptom management and comfort. They focus on quality care and set the standards for others to follow. Home care patients will see their nurse twice a week.

Dietitian:

Food and beverages play an important role in providing comfort at the end-of-life for many of our patient's. Hospice patients' dietary needs frequently change, in response to the disease process. Our Dietary Department, led by a Registered Dietitian and Certified Dietary Manager, focus on the patient's comfort by catering to individual preferences and dietary needs at a time that is right for them. Our patient centered "Comfort Kitchen" menu provides a variety of cultural and ethnic food choices and flexible eating times. Each patient is empowered to control this very important aspect of their care. Individual nutrition goals are honored by the entire interdisciplinary care team and Hospice staff.

Medical Social Worker:

Medical Social Workers work closely with patients and their loved ones to create and maintain a supportive care system that ensures safety and comfort. As needed, the medical social worker identifies community resources to help secure legal and financial assistance, teach personal coping techniques and provide information concerning advance directives.

Pastoral Care Department:

Our multi-denominational chaplains provide emotional and spiritual support for patients and loved ones and are available 24 hours a day, 7 days a week, with the highest chaplain to patient ratio in the country. A chaplain attends every death to assist the family in final preparations and is available for funeral services and grief support. Our In-patient care facility offers emotional comfort in two on-site chapels.

Financial Counseling Staff:

Our Patient Accounts staff is easily accessible to answer any questions relating to finances, coordinating and maximizing insurance coverage and general billing procedures.

Bereavement Counselors:

Bereavement counseling is provided for families and close friends to aid in the emotional healing of anticipatory grief. Following a loss, bereavement support is available as long as needed. We provide support groups, individual counseling, newsletters and telephone correspondence to give comfort. These services are available for all members of the community free-of-charge.

Integrative Therapies:

The Hospice of Saint John offers a wide range of integrative therapies to enhance patient care at the end-of-life. Aromatherapy, Animal-Assisted Therapy, Harp, Massage, Music and Sound Therapies, Healing Touch and Reiki are available for all our patients, free-of-cost. This innovative approach is research-proven to work in conjunction with traditional approaches to address pain, discomfort, dyspnea, pre-death anxiety, psychological and emotional stress, as well as many other symptoms.

We are the only hospice in the Denver area with a full-time Integrative Therapies Manager, Music Therapist and an Aromatherapist on site.

Professionally Trained Volunteers:

Volunteers are an essential part of our organization, complementing the quality care provided by staff, including personal care, companionship and support. Volunteers licensed in massage, hair care and other areas share their talent with patients and families.

Integrated Life Arts and Activites:

Our Integrated Life Arts and Activities Department adds a unique element of activities for patients and loved ones to enjoy. These include Art Class, Creative Writing Projects, Entertainment, Story Telling, Sculpting, Lasting Heritage Audio Program, Treasured Memories Photo Books, Dove Releases, Parties, Games and Field Trips.
